At your first appointment, you will receive a complete evaluation in order for us to understand what is causing your pain and/or limitations. Together, we will tailor a program to meet your goals. We understand that any pain is too much pain. Our goal is to address and correct your pain or symptoms.
Please fill out the appropriate paperwork for your condition before your first appointment. A new patient packet can be downloaded here in PDF format. The forms required include Conditions and Consent form, HIPAA consent form, new patient intake form and the Patient History form.

Physical therapy services are typically initiated once you have discussed your condition with your health care provider. Physical therapy is a conservative treatment approach for pelvic muscle dysfunction. If your health care provider is not familiar with this treatment we would be happy to provide information to them regarding our treatment protocols. In the state of Colorado, you have access to treatment by a physical therapist without a physician referral. Insurance guidelines may require a prescription for reimbursement. If you come to therapy without a previous medical assessment, we may request a visit to your healthcare provider if the therapist feels it is in your best interest. Discuss your condition with your health care provider and ask for a referral.
